Listed below are several forms that might be necessary for your son/daughters participation at a Eurotech® Residential Camp this summer. You have the choice to choose the Adobe Acrobat format, or view an image of the form via your browser and print it. The PDF format is preferred but not necessary if you do not have Adobe Acrobat Reader.

Once you have registered and received your confirmation it is important to print out and complete the following forms:
Required Forms for All Eurotech® Campers
The Medical Information & Liability Release Form concerning camper medical insurance and guardianship must be completed and signed by the camper’s legal parent or guardian. All campers must submit a fully completed Eurotech® Medical Form/Release prior to participation on the first day of camp. All campers requiring medication while at camp will also have to complete and submit an Authorization of Medical Dispensation Form. Please DO NOT submit or mail these forms to the Eurotech® Corporate Office prior to the start date of camp - please bring these completed forms with you to the camp registration from 12:00noon-2:00pm on the first day of camp. On-site Eurotech® Medical Staff will review the forms with you and you can speak to them directly about any additional medical concerns that you may have regarding your son/daughter.
No medication may be kept by the player or self-administered. At camp registration, all medications will be collected by the Trainer/Camp Director and provided to the player for administration as needed.
*Note: Campers will not be allowed to participate in camp without receipt of this fully completed Form!
Optional Forms for Teams
All attending teams are encouraged to complete the Eurotech® Team Profile Form and submit it to the on-site Eurotech® Camp Director during registration on the first day of camp from 12:00noon-2:00pm. The information on this valuable form will allow the Coach/Team Manager the opportunity to give the Eurotech® Trainer some detailed information about the team’s strengths and weaknesses and if their are any specific teaching topics that may need to be addressed or emphasized during the week of camp. This allows the team to optimize their training experience with Eurotech® and increase overall performance in a more efficient manner.

Required Forms for National Performance Center Participants
All NPC particpants must complete our Eurotech® Player Profile Form and send it in prior to the start of camp. This form will give our Director an overview of your son/daughter’s playing experience.
